. SCENARIO: • A 48 years old lady complains of fever and cough for last 3 days. She was alright 3 days back when she experienced sudden onset, high grade and continuous fever. She has cough which is productive of large amounts of dark yellow sputum. She has also been feeling increasing shortness of breath which is present during activity as well as rest. There is no effect of posture on it. She says her chest has become very noisy with crackling sounds. She has taken aspirin but there is no relief. She has been diagnosed with breast cancer 3 months ago for which surgery was done and she is now taking chemotherapy medicines for it. She is a smoker, smoking 1 pack per day for last 10 years She is on taking calcium supplements as well. She is a school teacher. She has 3 children. Her husband is a car mechanic. . On examination, Her temperature is 102 F, her pulse rate is 120/minute, Blood pressure is 130/80 and respiratory rate is 22/minute. On chest percussion, there is a dull note in the right lower zone.
