We are looking for a Cpt code that describe the situation of thepatient whether an established/New, impatient or outpatient, thethree elements will point at the code specifically.

+. A patient returns to the physician's office for continued cold-like symptoms. The patient complains of a runny nose and her cough has escalated with the production of a thick, greenish substance in addition to a fever. She states she has been unable to work for the past week. It is unrelieved by OTC meds and she would like further evaluation. A detailed exam was performed in addition to a detailed history. The presenting problem remained at moderate severity. Medical decision making was deemed low complexity. 5. An internal medicine physician requests an opinion from a cardiologist regarding his inpatient's chest pain. The cardiologist reviews the chart, all diagnostic tests and monitoring. The patient often got good relief of her chest pain with Maalox. The medical decision making was low level. An expanded problem focused history was obtained. The exam was detailed and included all related body areas and systems. The cardiologist reported his findings in the shared medical record.
